Our office is experienced in handling injuries resulting from an auto accident or work accident. If you are unable to complete the New Patient Information Form 3 prior to your visit, please bring the following applicable information: 

• Auto insurance claim number
• Auto insurance Adjuster’s name and phone number
• Adverse (3rd Party) Auto insurance claim number
• Adverse auto insurance Adjuster’s name and phone number
• Attorney contact, in applicable
• Worker’s Compensation claim number
• Worker’s Compensation Adjuster’s name and phone number 

• One-on-one consultation with a physician
• Review of symptoms, conditions or concerns
• Physical and orthopedic examination
• On-site radiography (x-ray)
• Additional diagnostic testing may be recommended (ie MRI, CT, lab work, etc)
• Review findings and treatment plan
• Provide referrals to other health care professionals if it is out of the scope of the practice

We provide affordable health care plans and accept most insurances to include MEDICARE, MEDICAID/AHCCCS, BLUE CROSS BLUE SHIELD, AETNA, CIGNA, GREAT WEST, HEALTH CHOICE, HEALTH NET, UNITED HEALTH CARE and many more. Call our office if you don’t see your insurance listed here. Reasonable payment schedules are also available for uninsured individuals.
